Extended indication Orencia in combination with methotrexate is indicated for the treatment of moderate to severe active polyarticular juvenile idiopathic arthritis (pJIA) in paediatric patients 2 - 6 years of age who have had an inadequate response to previous DMARD therapy. Orencia can be given as monotherapy in case of intolerance to methotrexate or when treatment with methotrexate is inappropriate.

Substantiation Abatacept wordt gezien als een veilig middel met weinig bijwerkingen en goede behandeloptie voor kinderen met JIA welke onvoldoende reageren op methotrexaat (of een intolerantie hebben ontwikkeld) en onvoldoende baat hebben bij anti-TNF gebruik. Bijkomend voordeel is dat het ook als infuus gegeven kan worden in het ziekenhuis, zodat subcutaan prikken thuis in deze lastig te prikken groep niet hoeft. Registratie voor kinderen <6 jaar voor abatacept zou mooie aanvulling zijn op de huidige behandelopties.

Additional remarks JIA komt bij ongeveer 4.000 kinderen in Nederland voor; 30-40 nieuwe JIA diagnoses per jaar. Remissie na de eerste of tweede lijn bedraagt ongeveer 70%. De inschatting is dat Orencia in de derde lijn zal worden ingezet. Gezien de indicatie bedoeld is voor kinderen <6 jaar is de inschatting dat dit 10-20 patiënten zal betreffen.
