Substantiation Bij therapieresistente of prednison afhankelijk CU bij kinderen wordt gestart met een biological. Hierbij heeft infliximab de voorkeur, zeker bij opgenomen patiënten met acute severe colitis. Ambulante patiënten waarbij er voor een biological wordt gekozen vanwege bijvoorbeeld actieve ziekte ondanks azathioprine, is adalimumab waarschijnlijk even effectief als infliximab, en kan adalimumab voordeel bieden voor de patiënt die dan de behandeling meer in eigen hand heeft (s.c. injecties). Wellicht zal adalimumab minder in trek zijn bij jonge pediatrische populatie vanwege het 2x per week subcutaan spuiten (t.o.v. bv infliximab 1x/8 weken op de dagbehandeling). Wanneer er sprake is van antistofvorming tegen infliximab, wordt ook adalimumab gestart bij kinderen met CU.

Additional remarks Colitis ulcerosa komt steeds vaker voor, met name in de westerse landen. Er zijn momenteel ruim 80.000 mensen in Nederland met een chronische darmontsteking. Hiervan heeft iets meer dan de helft colitis. De leeftijdsgroep van 4 tot 17 jaar betreft 17% van de Nederlandse bevolking. De verwachting is dat vooral ambulante patiënten waarbij er voor een biological wordt gekozen vanwege bijvoorbeeld actieve ziekte adalimumab waarschijnlijk even effectief is als infliximab. Wanneer er sprake is van antistofvorming tegen infliximab, wordt ook adalimumab gestart bij kinderen met CU. Volgens de inschatting van een expert zal het patiëntvolume 100 tot 250 patiënten bedragen.
