Primaire eindpunten van de MYSTIC trial voor de combinatie tremelimumab/durvalumab zijn niet gehaald. Deze registratie zal gebaseerd zijn op de POSEIDON trial. Registratie wordt verwacht in het eerste kwartaal van 2023. Voor durvalumab geldt tot eind 2023 een financieel arrangement voor oncologische indicaties.

In 2018 waren er 10.190 diagnoses NSCLC in Nederland. Dit betrof 1.724 stadium I, 921 stadium II, 2.452 stadium III (waarvan zo'n 55% stadium IIIa en zo'n 45% stadium IIIb), 5.062 stadium IV en 31 onbekend. Betreft stadium IV. Hier komen nog patiënten bij uit eerdere stadia die verslechteren naar stadium IV. Binnen een jaar is de inschatting dat dit 20% van stadium I/II bedraagt (529) en 50% van stadium III (1.226). In totaal bedraagt deze groep dus zo'n 6.850 patiënten. Van deze patiënten zal maximaal 65% een eerstelijnsbehandeling krijgen, dat komt neer op <4.500 patiënten. Deze behandeling zal zijn plaats hierin moeten veroveren. De fabrikant geeft aan zelf 296 patiënten te verwachten.

Tremelimumab wordt gegeven in dosering 75mg per cycle (voor de eerste 4 cycles). Durvalumab wordt in combinatie gegeven met pemetrexed. De kosten van tremelimumab en pemetrexed zijn in bovenstaande berekening niet meegenomen. De dosis durvalumab is 1.500 mg. Prijs per toediening is ( 3x €2.328) = €6.984. De eerste 4 cycli wordt dit elke 3 weken gegeven (met of zonder tremelimumab), daarna nog een keer de combi bij 16 weken (dus 5x de combinatie). (prijs tot 16 weken is dus 5 x €6.984 = €34.920). Daarna elke 4 weken durvalumab monotherapie. Voor 6 maanden is dit (26-16)/4 x €6.984 = €17.460 + €34.920= €52.380; voor 52 weken is dit €52.380+26/4x€6.984= €97.776.
