Therapeutic value

Current treatment options Patiënten met chronische nierschade en diabetes mellitus type 2 (DM2) worden in Nederland behandeld met een ACE-remmer of een ARB om progressie van chronische nierschade tegen te gaan. Vanwege bijwerkingen wordt spironolacton (off-label) toegevoegd aan een ACE remmer of ARB in zeer specifieke gevallen (patiënten met een ernstige albuminurie).

Additional remarks Diabetesfonds: al ruim 1,2 miljoen Nederlanders hebben diabetes en elke week komen daar 1.200 bij. Dit gaat om 62.400 Nederlanders per jaar. Gezien nierschade afhankelijk is van het aantal jaren dat iemand leeft met diabetes zullen veelal niet de nieuwe patiënten in aanmerking komen. Nierstichting: 28% van de mensen met diabetes heeft chronische nierschade. Inschatting is dat dit een latere lijnsbehandeling wordt en er mogelijk 100 tot enkele duizenden patiënten per jaar in aanmerking komen voor finerenone. De plaatsbepaling lijkt ongeveer hetzelfde te worden als van de SGLT-2 remmers (empagliflozine/dapagliflozine), dit geneesmiddel zal mogelijk daarnaast gegeven worden, maar dit is tot nu toe nog niet goed onderzocht.
