Substantiation Gepotidicin is effectief tegen E. coli en S. saprophyticus. Dat zijn beide belangrijks pathogenen bij urineweginfecties. In de EAGLE 2 is gepotidacin vergeleken met nitrofurantoine in vrouwen en adolescenten met een nitro gevoelige stam. Gezien het andere werkingsmechanisme ten opzichte van de reeds op de markt zijnde middelen en de orale toedieningsvorm is het een toevoeging aan de huidige behandelopties. Het geneesmiddel werkt op zowel DNAS gyrase als op topoisomerase IV (pol C). Helaas lijken stammen die ongevoelig zijn voor FQ hogere MIC waarden te hebben. Nog geen duidelijk inzicht in de gevoeligheden van nitro en/of fosfomycine resistente stammen, maar de eerste data zijn niet zo veelbelovend.

Additional remarks Er wordt verwacht dat in eerste instantie de bekende geneesmiddelen zoals nitro, fosfo, amoxi en cotrim ingezet zullen worden. Het is tot nu enkel in vrouwen bestudeerd, maar als er geen andere opties zijn zullen artsen het mogelijk ook aan mannen voorschrijven. De resistentie cijfers voor E coli en S saprophyticus zijn als volgt: nitro 2%, fosfo 3% en trim 25% (1). In 2021 waren er ongeveer 600.000 gebruikers van nitrofurantoine, 180.000 gebruikers van fosfomycine en 61.000 van trimethoprim (2). Er komen in theorie dus maximaal 42.000 patiënten in aanmerking.
