Therapeutic value

Current treatment options	Calcium oxalaat kristal remmers. In sommige gevallen niersteenbehandeling, dialyse en mogelijk lever of niertransplantatie. Lumasiran is in Nederland beschikbaar via ODAP. Er zijn subgroepen die goed reageren op pyridoxine (vitamine B6) en daarom niet met lumasiran of nedosiran behandeld hoeven te worden.

Substantiation	Lange termijn uitkomsten op klinisch relevante uitkomstmaten zijn nog onvoldoende bekend. RCT bij PH1 en PH2 patiënten met eindpunt daling van urine oxaalzuur spiegel (Uox). Significante daling van Uox en plasma oxolaat in nedosiran behandelde PH1 groep ten opzichte van placebo. Nota bene: geen consistent effect bij PH2 patiënten. Het lijkt gelijke waarde ten opzichte van lumasiran te hebben. De beroepsgroep verkent een tweede ODAP traject voor dit middel.

Additional remarks	In adults and in adolescents (aged 12 tot 17 years) weighing greater than or equal to (>=) 50kg, Nedosiran will be administered via subcutaneous injection once monthly at a dose of 170mg. In adults and adolescents weighing <50kg, nedosiran will be administered once monthly at a dose of 136mg. The dose for participants aged 6 to 11 years will be 3.5mg/kg monthly, not to exceed 170mg.
