Therapeutic value

Current treatment options Tigecycline en colistine voor HAP. Voor UWI is het ook een geschikt carbapenem sparend regime bij ESBL infecties.

Substantiation Voor HAP wordt plazomicine gecombineerd met een ander antibioticum zoals bijvoorbeeld meropenem (afhankelijk van het micro-organisme). Ten opzichte van colistine combinatie therapie laat een kleine studie een duidelijke betere survival zien. Verder is plazomicine een middel die als monotherapie kan worden gebruikt bij (gecompliceerde) UWI met resistente gram negatieve micro-organismen, inclusief ESBL (carbapenem-sparend). Temocolline is een mogelijk alternatief, maar naar verwachting zal plazomicin de concurrentiestrijd winnen. De toxiciteit bij niertransplantatie is onduidelijk.

Additional remarks Afgelopen jaren is het meropenem gebruik verviervoudigd, mede gedreven door ESBL infecties, met name urineweginfecties. Plazomicine heeft ook een plek voor UWI veroorzaakt door ESBL en niet alleen CRE. Incidentie urineweginfecties is 40-60/1.000 volwassen inwoners. ESBL rate is 10%. Er zijn echter ook nog andere (orale) alternatieven voor handen. Er wordt verwacht dat ongeveer 50 patiënten per ziekenhuis dit geneesmiddel zullen gebruiken. Dit komt neer op een patiëntvolume van rond de 4.000.
