Additional remarks In 2018 waren er 10.190 diagnoses NSCLC in Nederland. Dit betrof 1.724 stadium I, 921 stadium II, 2.452 stadium III (waarvan zo'n 55% stadium IIIa en zo'n 45% stadium IIIb), 5.062 stadium IV en 31 onbekend. De indicatie betreft patiënten met NSCLC stadium IV. Hier komen nog patiënten bij uit eerdere stadia die verslechteren naar stadium IV. Binnen een jaar is de inschatting dat dit 20% van stadium I/II bedraagt (529) en 50% van stadium III (1.226). In totaal bedraagt deze groep dus zo'n 6.850 patiënten. Het aantal patiënten dat geen plaveiselcelcarcinoom heeft betreft 77% (<5.275 patiënten). Uit studies blijkt dat HER2 mutaties bij zo'n 2 tot 4% voorkomen. Gezien het een indicatie betreft waarbij een mutatie een rol speelt is de verwachting dat van deze patiënten maximaal 85% een eerstelijnsbehandeling krijgen (90 tot 180). De aanbevolen dosering is 6,4mg per kg per behandelcyclus van 21 dagen. Op basis van een gemiddeld gewicht van 72 kg bedragen de kosten van trastuzumab-deruxtecan €9.000 per behandelcyclus (5 flacons van €1.800).
