Therapeutic value

Current treatment options Voor patiënten met gevorderd hodgkinlymfoom (Stadium III/IV HL), adviseert de richtlijn behandeling met escBEACOPP of de combinatie doxorubicin, bleomycin, vinblastine en dacarbazine (ABVD). In de praktijk gaat de voorkeur uit naar escBEACOPP gezien de betere kans op tumor controle en de kortere behandelperiode.

Substantiation De verwachting van de werkgroep is dat het veelbelovend gaat zijn en zal de eerste lijn gaan veranderen. De studie is opgezet op basis van gelijke waarde maar het is superieur in PFS en minder toxiciteit.

Additional remarks Door deze indicatieuitbreiding komen maximaal 200 patiënten extra in aanmerking voor behandeling met Br-ECADD (50 HL st IIB patiënten + 150 HL st III/IV). Jaarlijks krijgen ongeveer 448 patiënten in Nederland de diagnose hodgkinlymfoom, waarvan maximaal 200 patiënten HL stadium III / IV zijn. De richtlijn beveelt behandeling met escBEACOPP of ABVD aan voor gevorderd hodgkinlymfoom, waarbij in de praktijk de voorkeur uitgaat naar escBEACOPP gezien de betere kans op tumor controle en kortere behandelperiode. Maximaal 75% van de stadium III/IV patiënten komt in aanmerking voor escBEACOPP (de huidige standaardbehandeling) en met deze registratie voor Br-ECADD op basis van expert opinie: 150 patiënten (75%x200). Daarnaast zal Br-ECADD ook worden ingezet voor HL stadium IIB met grote mediastinale massa en/of extranodale lesies: 50 patiënten.

Additional remarks Een flacon met 50mg injectiepoeder kost €3.031,31. Gemiddeld gebruik studie: 4,8 cycli (PET gestuurd: 60% 4 cycli + 40% 6 cycli). Met een dosering 1,8mg/kg zijn maximaal 3 vials van 50mg per gift nodig. De kosten per patiënt per jaar bedragen dan €43.650,86 (4,8 cycli * 3 vials * €3.031,31). Deze bedragen staan los van de kosten voor Etoposide, Cyclophosphamide, Doxorubicin en Dacarbazine (Br-ECADD).
