Substantiation Eerste keus in de behandeling van therapie-naïeve volwassen patiënten met een HIV-1-infectie is meestal een tripeltherapie. Deze bestaat uit twee nucleoside reverse-transcriptaseremmers (NRTI’s) in combinatie met een derde actief antiretroviraal middel uit een van de volgende drie klassen: bij voorkeur een integraseremmer (INSTI) met een hoge barrière tot resistentie (bictegravir of dolutegravir) en in specifieke situaties met een non-nucleoside reverse-transcriptaseremmer (NNRTI) of een proteaseremmer (PI) met een farmacokinetische booster (cobicistat of ritonavir). In deze gewichtsklasse is zo een behandeling nog niet geregistreerd. Het zal daardoor voor deze patiënten wel een meerwaarde hebben. Het toedieningsgemak is ook prettig. Abacavir is wel geassocieerd met vaatschade. Abacavir wordt af en toe gegeven bij volwassenen. Een andere combinatie is daar de standaard.
