Additional remarks De hematopoietische stamcellen van een patiënt zullen worden geoogst en gemodificeerd (getransduceerd) met behulp van de Lenti-D lentivirale vector die codeert voor humaan adrenoleukodystrofie-eiwit. Na modificatie (transductie) met de Lenti-D lentivirale vector zullen de cellen opnieuw worden toegediend (getransplanteerd) aan de patiënt na myeloablatieve conditionering.

Additional remarks X-ALD is gelinkt aan het X-chromosoom, om die reden komt deze ziekte dan ook vrijwel alleen bij jongens voor. X-ALD komt voor bij 1-2,5 op de 50.000 jongens wat neerkomt op 170-425 patiënten in Nederland. Er zijn momenteel ongeveer 200 patiënten met X-ALD bekend in Nederland. Van deze groep ontwikkelt slechts een klein deel de cerebrale vorm. Op dit moment worden 2-5 patiënten per jaar getransplanteerd voor deze indicatie: dit zal de populatie zijn die in aanmerking komt voor gentherapie. De Gezondsheidraad heeft geadviseerd om X-ALD op te nemen in de hielprik. Dit zal mogelijk effect hebben op de patiëntenaantallen.

Additional remarks Waarschijnlijk betreft dit een dure behandeling. De kosten per patiënt per jaar worden zeer hoog. Bleubird heeft de intrinsieke waarde van de vergelijkbare behandeling met Zynteglo gesteld op $2,1 miljoen (€1,8 miljoen). De lijstprijs is nog niet bepaald maar zal volgens Bluebird niet boven dit bedrag uitkomen.
