Additional remarks Infigratinib (BGJ398) is an orally administered, ATP-competitive, FGFR1-3 tyrosine kinase inhibitor in development for the treatment of FGFR-driven conditions. Overactivating FGFR3 mutations drive downstream MAPK and STAT1 signaling that aberrates growth plate development, thereby causing disproportionate short stature and serious health complications stemming from cranial and spinal defects. Low dose infigratinib has the potential to help these patients due to its direct inhibition of the mutant FGFR3 receptor.

Substantiation Mogelijk zal Infigratinib een plaats kunnen krijgen binnen de behandeling. Het is echter op dit moment nog niet duidelijk hoe de behandeling er mogelijk uit zal komen te zien. In de fase 2 studie werden 72 kinderen variërend 3 tot 11 jaar oud behandeld in vijf cohorten gedurende maximaal 18 maanden. De primaire effectiviteitsuitkomst was de verandering ten opzichte van baseline in de groeisnelheid per jaar (in cohort 5 met 0,25 mg/kg 2,50 cm/jaar). Voordelen ten opzichte van de vosoritide behandeling (C-type natriuretic peptide (CNP) analog) qua effectiviteit (hogere groeisnelheid/jaar) en toedieningsvorm (oraal versus dagelijkse subcutane injecties). Vragen zijn onder andere hoe de langere termijn uitkomsten zijn. (Het eindeffect op volwassen lengte, met andere woorden, wat de kosten per cm gewonnen eindlengte mogen zijn), of vroegere behandeling gunstig is en de plaatsbepaling ten opzichte van andere FGFR3-selective geneesmiddelen die worden bestudeerd (TYRA-300; LY3866288 (LOXO-435), FORAGER-1 trial), FGFR-remmers die momenteel in klinische ontwikkeling zijn voor oncologische indicaties en die op termijn mogelijk gerepurposed kunnen worden voor achondroplasie.

Additional remarks De incidentie van achondroplasie wordt wereldwijd geschat op ongeveer 1 op de 25.000 levendgeborenen. Dit betekent dat er een prevalente pool van ongeveer 135 patiënten van 0 tot 17 jaar oud is. Door de fabrikant van vosoritide werden er zeven nieuwe patiënten verwacht met achondroplasie per jaar in Nederland op basis van een expertopinie.
