Therapeutic value

Current treatment options Na steroïden in de eerste lijn is de huidige standaard behandeling in de tweede lijn ruxolitinib. Diverse middelen worden toegepast in de derde lijnsbehandeling van GVHD waaronder calcineurineremmers, mesenchymale stromale cellen (MSC), etanercept, antithymocyte globuline (ATG), mycofenolzuur, Infliximab, rituximab, et cetera.

Substantiation In het verleden veel, voornamelijk fase 2, studies verricht met MSC bij GVHD patiënten met de meest gerapporteerde effectiviteit van rond de 50%. Daar MSC een ATMP betreft lijkt de werkzaamheid ook sterk samen te hangen met de oorsprong van de MSC, manier van kweken en gebruikte donoren. Mede hierdoor is een algemene uitspraak over de effectiviteit van MSC lastig.

Additional remarks Jaarlijks worden in Nederland circa 600 allogene stamceltransplantaties uitgevoerd. In naar schatting de helft van de gevallen (=300) treedt enige vorm van Graft-versus-Host-ziekte op welke behandeld wordt met steroïden. Hiervan is naar schatting 50% steroïde refractair, wat maakt dat jaarlijks in totaal 150 patiënten theoretisch in aanmerking zouden komen voor behandeling.
