Substantiation Migalastat is slechts zinvol voor een kleine groep Fabry patiënten met een "amenable" mutatie: een mutatie die leidt tot een instabiel enzym, dat door migalastat gestabiliseerd wordt. Dit zijn vaak 'missense' mutaties en de meerderheid van de patiënten heeft dan ook een milder fenotype (niet-klassiek of intermediair fenotype). Het natuurlijk beloop hiervan is milder. Kinderen komen pas in aanmerking voor behandeling als er eerste tekenen zijn van orgaanschade of als zij een ernstig fenotype hebben, zodat verwacht kan worden dat er op jonge leeftijd schade ontstaat. Migalastat is dus zeer waarschijnlijk niet zinvol op de kinderleeftijd, omdat er simpelweg dan nog geen orgaanschade is en jongens met een klassiek fenotype geen amenable mutatie zullen hebben.
