Niraparib-abirateron is in de sluis geplaatst (mei 2023), voor zover verstrekt in combinatie met prednison of prednisolon voor de geneeskundige behandeling van volwassen patiënten met gemetastaseerde castratieresistente prostaatkanker en BRCA1/2-mutaties bij wie er geen klinische indicatie is voor chemotherapie. Op verzoek van de minister van Volksgezondheid, Welzijn en Sport (VWS) beoordeelt Zorginstituut Nederland of niraparib-abirateron (Akeega®) vergoed kan worden uit het basispakket van de zorgverzekering. Start beoordeling dossier november 2023. Publicatie advies is nog niet bekend. Substantiation Resultaten uit de (MAGNITUDE) studie zijn in februari 2022 gepubliceerd. Toepassing in de eerste lijn zal er komen, de vraag is welke PARP-remmer het meest effectief zal zijn. De mediane rPFS in de BRCA1/2 subgroep was significant langer in de niraparib + AAP groep vergeleken met placebo + AAP groep (16,6 versus 10,9 maanden; hazard ratio [HR], 0.53; 95% CI, 0.36 to 0.79; P = .001). Er is echter nog geen overall survival data beschikbaar. Het primaire eindpunt rPFS is niet erkend als betrouwbaar eindpunt (in Nederland). Hiervan komt naar schatting 67% (n=2.734) in aanmerking voor een levensverlengende therapie (2). Een deel van deze patiënten (ongeveer een kwart) wordt al behandeld met abirateron wanneer het mHSPC betreft (het voorstadium van mCRPC) en zal hierna niet opnieuw abirateron krijgen in combinatie met niraparib (3). Uitsluitend mannen met uitgezaaide castratieresistente prostaatkanker (mCRPC) bij wie tevens sprake is van een mutatie in de BRCA1 of -2 genen (BRCA1/2+ mCRPC) komen volgens het label in aanmerking voor deze behandeling.
