Substantiation Het plasma TG gehalte is een voorspeller van HV ziekten en het ligt voor de hand dat het laten dalen van TG de kans daarop zou verminderen. Dat is tot nu toe echter niet gebleken. Fibraten lijken geen effect op CV prognose te hebben hoewel ze de TG wel verlagen. Het zou kunnen dat andere manieren van verlagen van TG wel een CV benefit heeft, maar dat vereist CV outcome studies. Fibraten worden wel gebruikt om het risico op pancreatitis door hypertriglyceridemie te verlagen. Er zijn patiënten met extreem verhoogd TG, de dd daarvan bevat dan ook genetische defecten - als er sprake is van verhoogde chylomicronen is dat familiaire hyperchylomicronemie op basis van complete LPL-deficiëntie en apoCII-deficiëntie. Er zijn twee centra waar deze patiënten gezien worden in centrum erfelijke zeldzame dyslipidemien (AMC, ErasmusMC). Het RadboudUMC is nog in aanvraag. De meeste patiënten zijn niet therapietrouw en komen als ze pancreatitis hebben. Afhankelijk hoe het monitoringsschema voor thrombo's wordt (indien hetzelfde als voor volenesorsen dan is het verwachtte aantal 10 patiënten). Indien het minder frequent hoeft dan 20 tot 30 patiënten.
