Additional remarks In LIBRETTO-001 waren de meest voorkomende RET-fusie positieve tumoren anders dan long- of schildkliertumoren: pancreas, colorectaal, salivary, sarcoma en onbekend primair. De indicatie in het verwachte label zal alleen betrekking hebben op patiënten met een RET-fusie. Patiënten met een RET-mutatie komen dus niet in aanmerking voor behandeling met selpercatinib. De incidentie van RET-fusie positieve solide tumoren anders dan NSCLC en TC is 0,0863%(1-6). Het voorkomen van RET-fusies komt overeen met dat van NTRK-genfusies (0,3 tot 1% van alle vergevorderde of gemetastaseerde solide tumoren) (7). In het Consensusrapport Diagnose en Behandeling van NTRK-genfusie gerelateerde solide tumoren (7) wordt gesteld dat "er in Nederland circa 110.000 nieuwe oncologiepatiënten per jaar zijn, waarvan circa 60% na behandeling geneest". De overige 40% vertoont na verloop van tijd lokale uitbreiding en/of metastasering (circa 44.000). Verwacht wordt dat hiervan ongeveer 25.000 patiënten een goede conditie zullen hebben om een behandeling met een TRK-remmer te kunnen ontvangen. Deze patiënten komen in aanmerking voor een screenende test. Dit zou betekenen dat 22 patiënten (0,0863% van 25.000) met een RET-fusie gevonden zullen worden.
