Er is namelijk geen landelijk register en daarnaast zijn er meerdere mogelijkheden in de lijnen na de eerste lijn. De keuzes die gemaakt worden zijn vaak patiënt afhankelijk en de verdeling binnen de lijnen daardoor zeer moeilijk te voorspellen, los van het feit dat in de richtlijn wel duidelijk de diverse opties worden beschreven. Veel van de behandelingen sluiten elkaar uit - dat betekent dat niet alle mogelijkheden benut/gebruikt zullen worden, hetgeen betekent dat het aantal patiënten dat in aanmerking komt sterk kan verschillen van het aantal patiënten dat daadwerkelijk de therapie krijgt.
