Het is ontwikkeld om te hechten aan het genetische materiaal van de cel die verantwoordelijk is voor het produceren van het eiwit genaamd ICAM- 1, waardoor de productie ervan wordt geblokkeerd. ICAM-1 bevindt zich op het oppervlak van leukocyten en aan de binnenkant van bloedvaten en is betrokken bij het ontstekingsproces dat wordt gezien bij pouchitis. Door de productie van ICAM-1 te blokkeren, wordt verwacht dat alicaforsen de ontsteking vermindert en de symptomen van de aandoening verlicht.
