Ongeveer 42% (N=825) heeft op moment van diagnose of in een later stadium (met/zonder nefrectomie) stage IIIb/IV RCC (Lam et al. 2005). De prognose van de patiënten bepaalt de behandelkeuze. Aannemelijk is dat avelumab + axitinib net als sunitinib (vergelijkende behandeling pivotal trial) met name gebruikt zal worden voor patiënten met een goede of intermediaire prognose (78%; N=644) (Motzer et al. 1999). Deze patiënten komen in aanmerking voor avelumab + axitinib, echter zal een gedeelte niet behandeld worden (18%) en een gedeelte zal meedoen aan klinische studies (10%) (Pakketadvies bevacizumab). Het resterende gedeelte (n=459) kan met de huidige behandelopties (o.a. sunitinib of pazopanib ) of met 1 van de 3 PD(L)1 remmers waaronder avelumab behandeld worden. Het marktaandeel zal afhangen van de nog onbekende therapeutisch waarde van avelumab + axitinib t.ov. de huidige standaard therapieen en de nieuw te registreren (PD(L)1 combinatie therapieën.
