Het bindt dus specifiek ticagrelor waardoor het effect van ticagrelor wordt geneutraliseerd. Naar verwachting werkt het alleen bij ticagrelor en zal het alleen gebruikt worden bij patiënten die ticagrelor hebben gehad. Bentracimab wordt in een studie zonder controle groep onderzocht voor patiënten met ticagrelor die een chirurgische interventie ondergingen of een levensbedreigende bloeding hadden. De interim analyse laat gunstige klinische eindpunten zien. Helaas is er geen controle groep met het geven van bloedplaatjestransfusie gebruikt, dus de meerwaarde kan niet worden beoordeeld te opzichte van standard of care. Een probleem kan zijn dat het door chirurgen vaker zal worden gegeven dan geïndiceerd (bijvoorbeeld bij een geplande ingreep en er vergeten is de ticagrelor stop te zetten). Volgens het farmacotherapeutisch kompas worden ernstige bloedingen als bijwerking genoemd bij 1-10% van de gebruikers van ticagrelor (400 tot 4.000 patiënten). Daarnaast zal het bij acute operaties/interventies worden toegepast. Het is de vraag of al die patiënten wel in aanmerking komen voor bentracimab, met andere woorden of de bloeding wel ernstig genoeg is. Ticagrelor twee dagen van te voren stoppen, zorgt ook voor beëindiging effect. Bij geplande operaties is bentracimab dus niet nodig. De verwachting is dat het waarschijnlijk weinig ingezet zal gaan worden in Nederland (2).
