Dit betekent dat er een ‘wait & see policy‘ (3 tot 6 maanden) is (NHG-standaard). Na zes maanden volgt dan verwijzing naar dermatologie, die doen diagnostiek (met name uitsluiten andere auto-immuun ziekten zoals M. Hashimoto (schildklier), vitiligo, allergie, maar ook onderscheid met alopecia androgenetica). In de studies zijn alleen patiënten boven de 18 jaar geïncludeerd. Patiënten boven de 75 zullen over het algemeen niet behandeld worden. Jaarlijkse incidentie Alopecia Areata is 2,5% = 4.318 patiënten (huidige populatie = 17.487.491(1). incidence during life time = 2%(2); gem. levensverwachting = 81 jaar (3). Gemiddeld 35% komt in aanmerking voor behandeling (na geen spontane verbetering binnen 6-12 maanden) = 1.511. 1% heeft een JAKi-contra-indicatie = 1.496. Voor de behandeling zullen in principe alleen patiënten tussen 18-75 jaar in aanmerking komen = 1.122 (4). Aangezien het een lokaal geneesmiddel betreft zal het sneller ingezet worden dan systemische geneesmiddelen bij deze indicatie. Behandeling zal met name plaatsvinden bij grote psychische last.
