Therapeutic value

Current treatment options Colistine. Indicatie voor onderhoudsbehandeling met inhalatieantibiotica bestaat alleen bij drie of meer exacerbaties per jaar én pseudomonas in het sputum (Polverino, ERS richtlijn 2017).

Substantiation In september 2024 werden de resultaten van de PROMIS-I en PROMIS-II studies gepubliceerd (Haworth, Lancet Respir Med 2024). De PROMIS-I laat een 39% reductie in exacerbaties zien met dit colistine product versus placebo, terwijl de PROMIS-II geen verschil laat zien, maar vroegtijdig werd beëindigd vanwege de COVID19 pandemie. De studie concludeert dat colistine methaat natrium effectief en veilig is bij patiënten met bronchiëctasieën, pseudomonas en frequente exacerbaties.

Additional remarks De prevalentie van bronchiëctasieën in Nederland bedraagt 52 tot 67 per 100.000. Dat zijn ongeveer 9.000 tot 12.000 patiënten in totaal. Daarnaast telt Nederland ongeveer 1.600 patiënten met CF. Het aantal niet-CF bronchiëctasieën komt daarmee op 7.300 tot 11.400. Chronische infectie met P. aeruginosa komt voor in 12 tot 27% van de volwassen patiënten met bronchiëctasieën (876 tot 3.078). Voor dit middel komen echter alleen patiënten in aanmerking die tenminste twee oraal behandelde of één intraveneus behandelde exacerbatie in het afgelopen jaar hebben gehad. In het EMBARC cohort van bronchiëctasieën voldeed ongeveer 50% van de patiënten hieraan. Dus zou het gaan om 438 tot 1.539 patiënten.
