Substantiation In de AD-301 (NCT02118766) & AD-302 (NCT02118792) trials is aangetoond dat crisaborole meer effectief (pruritis) is dan vehikel alleen. Op dit moment wordt crisaborole onderzocht in de NCT03539601 trial en wordt de effectiviteit vergeleken met TCIs & TCSs. Vergelijkbaar met klasse 1-2 lokale steroïden, mogelijk steroïd-sparend effect. Maximum patient volume for sluice References Expert opinie Additional remarks Indicatiegebied crisaborole is mild tot matig eczeem, dit in tegenstelling tot dupilumab (matig tot ernstig eczeem). In Nederland was de incidentie (in 2015 145.000 patiënten 2 jaar en ouder (volksgezondheid.info). 85% (n=123.000 ) hiervan heeft mild tot matige atopische dermatitis (marktonderzoek). De behandeling van atopische dermatitis verloopt stapsgewijs afhankelijk van de ernst van de ziekte. Volgorde: emollient, TCS, TCI en systemische behandeling. Mild tot matig eczeem kan in principe goed behandeld worden met deze middelen, maar gezien de corticofobie bij veel patiënten , apothekers en behandelaars is het mogelijk dat crisaborale toch frequent wordt voorgeschreven. Het is dus nu onduidelijk hoeveel van de 123.000 patiënten die mogelijk in aanmerking komen crisaborole daadwerkelijk zullen gaan ontvangen. Er gaan ongeveer 120 FTU's in een tube, uitgaande van 0,5 g per FTU. Gebruik sterk afhankelijk van de aangedane huidoppervlakte. Maar een tube per maand lijkt niet onrealistisch ($8.800 in totaal). Waarschijnlijk zullen de kosten in Europa lager zijn. De prijsstelling zal niet vóór het tweede kwartaal van 2019 beschikbaar zijn. De prijs wordt (mede) vastgesteld op basis van de therapeutische waarde tov andere beschikbare therapieën. Hierbij zal ook rekening worden gehouden met de prijzen van huidige beschikbare behandelopties. Crisaborole kan worden ingezet als steroid-sparende behandeling bij onderhoudsbehandeling CE. Hiervoor wordt momenteel vaak Protopic gebruikt (€64,31 pppj in 2017). Prijsstelling dus erg afhankelijk van effectiviteit ten opzichte van Protopic.
