Substantiation De gepubliceerde fase 1 trials laten zien dat HIF-PHI therapie even effectief is als standaard EPO in het managen van Hb-gehaltes in CKD patiënten (al of niet dialyse afhankelijk). Potentiële voordelen van HFI-PHI zijn: orale toediening, lage EPO spiegels met een puur theoretisch voordeel op het optreden van cardiovasculaire events, onderdrukking van hepcidine en daarmee een betere ijzermobilisatie en een positief effect op lipiden (dapro en roxa). De groep van HFI-PHI werken mechanistisch identiek maar hebben andere effecten op celniveau; deze non-erythropoëtische effecten zijn, echter nog niet goed omschreven. Op het gebied van renale anemie therapie is er op dit moment geen duidelijke meerwaarde ten opzichte van EPO therapie; effecten anders dan anemie-gerelateerd zijn onvoldoende onderzocht. Voorlopig zullen nefrologen deze middelen niet veel voorschrijven omdat de lange termijn safety data ontbreken en er nog geen meerwaarde is aangetoond ten opzichte van EPO. De verwachting is dat deze vier producten allemaal op een marktaandeel uitkomen van rond de 5% (1.151 patiënten).
