Extended indication Asthma EXDENSUR is indicated as add-on maintenance treatment for severe asthma with type 2 inflammation characterised by blood eosinophil count in adults and adolescents 12 years and older who are inadequately controlled despite high dose inhaled corticosteroids (ICS) plus another asthma controller (see section 5.1). Chronic rhinosinusitis with nasal polyps (CRSwNP) EXDENSUR is indicated as an add-on therapy with intranasal corticosteroids for the treatment of adult patients with severe CRSwNP for whom therapy with systemic corticosteroids and/or surgery do not provide adequate disease control.

Substantiation Depemokimab is een afgeleide van mepolizumab en grijpt onder andere in op dezelfde epitoop van IL-5 als mepolizumab. Er hoeft minder medicament gebruikt te worden, namelijk slechts twee keer per jaar in tegenstelling tot ongeveer twaalf keer per jaar bij mepolizumab. De ultra-langwerkende eigenschappen en het tweemaal per jaar doseringsschema kunnen voordelen bieden op het gebied van patiënten trouw en gemak.

Additional remarks In 2015 waren er naar schatting 613.500 personen met astma. Op basis van farmaciegegevens waren er 13.250 patiënten met ernstige astma. Bij ongeveer 70% van de patiënten vindt de diagnose ernstige astma plaats, 9.275 (1). 50% (3.710 patiënten) heeft non-eos fenotype. 50% (3.710 patiënten) heeft het eos fenotype: De schatting is dat er ongeveer 2.000 tot 2.500 patiënten in aanmerking komen in de eerste jaren. Er is grote concurrentie tussen reeds vergoede biologicals geïndiceerd voor eosinofiel ernstig astma. Er wordt een aandeel verwacht van maximaal 20% à 742 patiënten door de fabrikant. Een doseringsfrequentie van twee keer per jaar zal veel aantrekkelijker zijn voor patiënten waardoor zij wellicht ook graag willen switchen. De uiteindelijk prijs zal echter ook belangrijk zijn, mocht de prijs afwijken van middelen die vaker per jaar gebruikt moeten worden. Dan zal dit invloed hebben op het patiëntvolume.
