Substantiation In de fase 3 studie CUPID A zijn 138 patiënten gerandomiseerd naar dupilumab (600mg oplaaddosering gevolgd door 300 mg iedere twee weken) of placebo. De patiënten in CUPID A waren niet eerder behandeld met omalizumab. Het primaire eindpunt voor Europa was de verbetering in UAS7 (urticaria activity score) op week 24. De UAS7-score verbeterde met gemiddeld 20,5 punten in de dupilumab arm, vergeleken met 12,0 punten in de placebo arm (p=0,0003). Verder bereikte 31,4% van de patiënten in de dupilumab arm een complete response (UAS7=0), versus 13,2% in de placebo arm. (Maurer et al. 2024). De data van CUPID C (n=151 patiënten, ook omalizumab naief) zijn nog niet gepubliceerd, maar wel bekend gemaakt in een persbericht en bevestigen de significante resultaten van CUPID A. Additional remarks Patienten met CSU worden op dit moment allereerst behandeld met niet-sederende antihistaminica, eventueel tot viermaal de standaard dosering. Bij onvoldoende respons wordt omalizumab gegeven (Nederlandse richtlijn). Andere mogelijke behandelingen zoals ciclosporine, methotrexaat, dapson en azathioprine zijn off-label. De mogelijke indicatie voor dupilumab betreft patiënten met matige tot ernstige CSU die gefaald hebben op antihistaminica en nog niet zijn behandeld met anti-IgE (omalizumab) therapie. Inmiddels is er een biosimilar van omalizumab in Nederland op de markt. Door de jarenlange ervaring met omalizumab als behandeling voor CSU en de lagere kosten van omalizumab (biosimilar) zal de eerste keus in de richtlijn na antihistaminica nog steeds omalizumab blijven. Dupilumab zal volgens de beroepsgroep slechts ingezet worden voor patiënten die naast CSU andere type 2 co-morbiditeiten hebben waar omalizumab niet voor geregistreerd is (met name atopische dermatitis).

Additional remarks Volgens de publicatie van Soegiharto R, et al (1) is de puntprevalentie voor chronische spontane urticaria (CSU) in Nederland 0,3% in 2018 wat neerkomt op 46.000 patiënten (12 jaar en ouder). Hiervan wordt ongeveer 37% behandeld (2). De eerste stap in de behandeling van CSU bestaat uit de toepassing van tweede generatie antihistaminica waarvan de dosering indien nodig (als 2e stap) opgehoogd kan worden tot maximaal viermaal de geregistreerde dosis (3). Uit de studie van Van den Elzen et al (4) blijkt dat 54% van de patiënten niet onder controle komt met een dosering tot maximaal viermaal de geregistreerde dosering, hiervan heeft 50% matige tot ernstige CSU (5). In Nederland worden volgens experts ongeveer 2400 CSU patiënten behandeld met omalizumab. Slechts 7% van de CSU patiënten heeft atopische dermatitis en zal volgens de plaatsbepaling van de beroepsgroep in aanmerking komen voor het gebruik van dupilumab. Dit komt neer op (4.500-2.400)*7% = ongeveer 150 patiënten.
