De prevalentie wordt geschat op ongeveer 50 per 100.000. De schattingen van het aantal mensen in Nederland dat de ziekte heeft lopen uiteen van 5.000 tot 8.000 patiënten (cijfers 2012 tot en met 2013). Deze marge is vrij breed omdat goed epidemiologisch onderzoek ontbreekt. In sommige streken, bij sommige etnische groepen en in sommige families komt sarcoïdose echter vaker voor dan in andere. Zo worden negroïde jonge vrouwen (40 op 100.000), alsmede Scandinavische (64 op 100.000), Ierse, Aziatische, Duitse, en Puerto Ricaanse bevolkingsgroepen vaker getroffen door sarcoïdose. Ook is het verloop van de aandoening bij deze groepen vaak agressiever. Dit doet vermoeden dat, hoewel duidelijke erfelijke factoren ontbreken, een relatie met bepaalde genen mag worden verondersteld, ook al zijn daar nog geen harde bewijzen voor. Van de 5.000 tot 8.000 hoeft het merendeel niet behandeld te worden met zware anti-inflammatoire middelen zoals prednison of MTX. Het gebruik zal dus lager uitvallen.
