Homozygote FH is een heterogeen ziektebeeld. De meeste patiënten hebben een aanvullende behandeling nodig naast statine en ezetimibe. Dit kan afhankelijk van type homozygote FH bestaan uit PCSK9-remming en/of lomitapide. Een klein deel van de homozygote FH patiënten wordt behandeld met aferese omdat dit een invasieve behandeling is. Een goede aanvulling op het huidige behandelassortiment. Evinacumab heeft een mogelijke meerwaarde ten opzichte van PCSK9-remmers. Het werkt enkel bij patiënten die LDL-receptoren hebben. Als een patiënt deze niet meer heeft is de enige optie seriële (wekelijkse of tweewekelijkse) lipoproteïne-aferese of het duurdere lomitapide. Het patiëntvolume was eerder geschat op 10 tot 20 patiënten (1). Dit is de uitbreiding van de indicatie voor patiënten van zes maanden tot vijf jaar. De ziekte wordt al vroeg opgemerkt door xanthoomvorming. Een schatting is dat 40 tot 60% van alle nieuwe diagnoses binnen deze leeftijd wordt gemaakt, wat neerkomt op vier tot twaalf patiënten (2).
