Substantiation Op dit moment is er nog geen goede causale behandeling voor C3G, waarbij activatie van de constitutionele alternatieve complement pathway door genetische of verworven afwijkingen in het complementsysteem een belangrijke rol speelt. Pegcetacoplan subcutaan 2x/week kan als remmer van C3 en C3b door remming van alternatieve complement pathway potentieel de complement gemedieerde schade bij C3G mitigeren en daarmee achteruitgang van nierfunctie afremmen en dialyse uitstellen of zelfs voorkomen. Dat zou ook kunnen gelden voor recidief na niertransplantatie dat bij overgrote deel van de C3G patiënten verantwoordelijk is voor het verlies van de transplantaat nier <5 jaar. In fase 2 studie (KI reports 2023) bij 8 patiënten van 16 jaar en ouder met C3G met eGFR >60ml/min/1.73m2 bleek pegcetacoplan na 48 weken proteïnurie met 51% te verlagen in ITT populatie en 65% in de per protocol populatie, die >75% van de giften had ontvangen. 75% van de patiënten had stabiele of verbeterde nierfunctie. Bij alle patiënten werd normalisatie van C3 gezien en forse afname van terminaal complement C5b-9. Gezien noodzaak van subcutane toediening lijkt de orale remmer van factor B Iptacopan een aantrekkelijker optie voor behandeling van C3G.
