Op 16 september 2022 heeft het Zorginstituut een pakketadvies uitgebracht over pegcetacoplan. Tijdens de beoordeling zijn het Zorginstituut en het Ministerie van Volksgezondheid, Welzijn en Sport tot de conclusie gekomen dat op basis van de afbakeningsbrief pegcetacoplan als extramuraal middel moeten worden aangemerkt. In april 2023 is voor dit middel een financieel arrangement overeengekomen. Met ingang van 1 mei 2023 is het geneesmiddel voor deze indicatie opgenomen in het GVS. (zie Staatscourant nr. 11871) Therapeutic value Current treatment options Bij een ernstige hemolyse onvangen patiënten een eculizumab biosimilar of ravulizumab. Substantiation Op het EHA congres juni 2020 is de PEGASUS fase 3 studie (randomisaie tegen eculizumab) gepresenteerd. Conclusie: "In this phase 3 trial in pts with PNH, pegcetacoplan demonstrated superiority to ECU in hemoglobin level, and improved clinical outcomes at week 16 with transfusion avoidance in most patients. The safety profile of pegcetacoplan was comparable to ECU. The efficacy of pegcetacoplan validates the prevention of extravascular as well as intravascular hemolysis in PNH, leading to a potential new therapeutic option." De PEGASUS trial was gedaan in patiënten die ondanks behandeling met eculizumab een blijvende hemolyse en laag Hb hadden. Of de superioriteit dus geldt voor alle patiënten met PNH is niet bekend. Het advies van het Zorginstituut over eculizumab laat een gestage groei zien, van 58 patiënten in 2015, 65 in 2016 naar 72 in 2017. Declaratiegegevens laten zien dat eculicumab bij PNH in 2017 gedeclareerd is voor 78 patiënten. De verwachting is dat de groei in het aantal patiënten min of meer overeen zal komen met de gestage groei die uit de declaratiegegevens voor eculizumab. References G-standaard Additional remarks Pegcetacoplan zal in 2e lijn gegeven worden aan die patiënten die nog niet afdoende onder controle zijn met eculizumab of ravulizumab. Prijs per flacon pegcetacoplan is €3.010, dit komt met 104 flacons per jaar neer op €313.040 per patiënt per jaar. Prijs is dus vergelijkbaar met eculizumab en ravulizumab.
