Substantiation Ruxolitinib crème is voor mild tot matig eczeem, wordt lokaal (op de huid) aangebracht en kent in de Verenigde Staten de indicatie dat andere lokale middelen niet effectief of gecontra-indiceerd zijn. Er is grote behoefte aan nieuwe lokale, corticosteroïde sparende geneesmiddelen, met name op gevoelige huiddelen zoals gelaat en lichaamsplooien. De lokale calcineurine remmers (protopic en elidel) geven nogal eens bijwerkingen. De verwachting is dat de lokale JAK/TYK remmers het beter doen (deze zijn al op de markt in Japan). Studies laten goede resultaten zien. Het is vergelijkbaar met effect klasse 2 à 3 corticosteroïden. Plaatsbepaling pas na corticosteroïden en calcineurine remmers. Gunstig bijwerkingenprofiel. De behandeling van atopische dermatitis verloopt stapsgewijs afhankelijk van de ernst van de ziekte. Volgorde: emollient, TCS, TCI en systemische behandeling. Mild tot matig eczeem kan in principe goed behandeld worden met deze middelen. Het is nog onduidelijk hoeveel van de 123.000 patiënten mogelijk in aanmerking komen. Er wordt aan de hand van gebruikersaantallen van Protopic verwacht dat er in theorie maximaal 50.000 patiënten in aanmerking komen indien er substitutie plaats zal vinden.
