Substantiation Er lijkt een potentiële plaats voor deze combinatie bij korte, pijnlijke procedures op de spoedeisende hulp, vooral omdat intra-nasale toediening van ketamine en fentanyl daar al gangbaar is. De meerwaarde zou met name liggen in het aantoonbaar verbeteren van analgesie ten opzichte van de huidige opties. Een praktische beperking is dat de commercieel beschikbare fentanyl-neusspray een vaste dosis van 50 microgram bevat, wat voor jonge of kleine kinderen vaak te hoog is; daarom wordt in de praktijk teruggegrepen op intraveneuze fentanyl die met een verstuiver nasaal wordt toegediend. Een formulering met een geschiktere concentratie zou dus relevant kunnen zijn voor pediatrisch gebruik. Kinderartsen passen intra-nasale fentanyl soms toe bij doorbraakpijn, maar het nadeel is het risico op snelle afhankelijkheidsontwikkeling.
