Extended indication Talzenna is indicated as monotherapy for the treatment of adult patients with germline BRCA1/2 mutations, who have HER2-negative locally advanced or metastatic breast cancer. Patients should have been previously treated with an anthracycline and/or a taxane in the (neo)adjuvant, locally advanced or metastatic setting unless patients were not suitable for these treatments. Patients with hormone receptor (HR)-positive breast cancer should have been treated with a prior endocrine-based therapy, or be considered unsuitable for endocrine-based therapy.

Additional remarks Incidentie invasieve borstkanker 14.497 patiënten (cijfersoverkanker.nl). Ongeveer 5% (725 patiënten) worden met een stadium IV tumor gediagnosticeerd (IKNL 2014,Pakketadvies palbociclib (Ibrance®)). Vervolgens zal naar schatting ongeveer 10% gBRCA positief zijn (72 patiënten) waarvan ongeveer de helft zal worden getest op BRCA1/2 mutaties (36 patiënten). Van de patiënten met een stadium I-IIIB tumor, zal ongeveer 20% uiteindelijk metastases (2.754 patiënten) krijgen (Pakketadvies palbociclib (Ibrance®)), waarvan na schatting 5% gBRCA positief zijn (137 patiënten). Ook hier zal ongeveer de helft worden getest op mutaties (68 patiënten). Van het totaal aantal gBRCA positief geteste patiënten (36+68 = 105 patiënten) zal naar verwachting van de fabrikant zo'n 90% (94 patiënten) in aanmerking komen voor behandeling met talazoparib. Expert opinie: deze aantallen kunnen mogelijk wel gaan toenemen als HDR patiënten ook deze behandeling gaan krijgen.

Additional remarks De fabrikant geeft aan dat de prijs van Talzenna in Q4 2019 gepubliceerd zal worden. De prijs wordt (mede) vastgesteld op basis van de therapeutische waarde t.o.v. andere beschikbare therapieën. Hierbij zal rekening worden gehouden met de prijzen van huidige beschikbare behandelopties. Zal derhalve de concurrentie aan moeten gaan met meerdere andere PARP-remmers zoals olaparib, veliparib en rucaparib. Olaparib voor een vergelijkbare indicatie staat ingeschat op €43.000,00.
