Extended indication Talzenna is indicated as monotherapy for the treatment of adult patients with germline BRCA1/2 mutations, who have HER2-negative locally advanced or metastatic breast cancer. Patients should have been previously treated with an anthracycline and/or a taxane in the (neo)adjuvant, locally advanced or metastatic setting unless patients were not suitable for these treatments. Patients with hormone receptor (HR)-positive breast cancer should have been treated with a prior endocrine-based therapy, or be considered unsuitable for endocrine-based therapy.

Additional remarks Incidentie invasieve borstkanker 14.497 patiënten (cijfersoverkanker.nl). Ongeveer 5% (725 patiënten) worden met een stadium IV tumor gediagnosticeerd (IKNL 2014,Pakketadvies palbociclib (Ibrance®)). Vervolgens zal naar schatting ongeveer 10% gBRCA positief zijn (72 patiënten). Van de patiënten met een stadium I-IIIB tumor, zal ongeveer 20% uiteindelijk metastases (2.754 patiënten) krijgen (Pakketadvies palbociclib (Ibrance®)), waarvan na schatting 5% gBRCA positief zijn (137 patiënten). Dit komt dus neer op zo'n 200 patiënten die maximaal voor deze behandeling in aanmerking kunnen komen. De verwachting is dat de incidentie van screening zal toenemen, om die reden geen rekening gehouden met aantallen patiënten die nu worden getest.

Additional remarks De fabrikant geeft aan dat de prijs van Talzenna in Q4 2019 gepubliceerd zal worden. De prijs wordt (mede) vastgesteld op basis van de therapeutische waarde ten opzichte van andere beschikbare therapieën. Hierbij zal rekening worden gehouden met de prijzen van huidige beschikbare behandelopties. Zal derhalve de concurrentie aan moeten gaan met meerdere andere PARP-remmers zoals olaparib, veliparib en rucaparib. Olaparib voor een vergelijkbare indicatie staat ingeschat op €43.000,00.
