Substantiation Een studie die gepubliceerd werd in het NEJM toonde aan dat een groep patiënten met hartfalen en behouden ejectiefractie die met tirzepatide behandeld werd een verminderde kans op cardiovasculaire dood of verslechtering van hartfalen had 9,9% versus 15,3% (P=0,026). De vraag is nog of dit veroorzaakt wordt enkel door gewichtsreductie of dat er andere pathways aan ten grondslag liggen. Ongeveer de helft van de patiënten heeft hartfalen met een behouden ('preserved') of matige ('midrange') ejectiefractie (HFpEF/HFmrEF, kleiner dan 40%). Er is een grote groep waarbij het hartfalen met behouden LV-functie nog niet is gediagnosticeerd. De inschatting is dat de helft van de totale groep patiënten met hartfalen in aanmerking zal komen voor dit geneesmiddel (2). Bij dit middel gaat het om patienten met BMI hoger dan 30.
