Additional remarks Max. patiëntenaantal is gebaseerd op werkgroep en eerder pakketadvies pembrolizumab van Zorginstituut Nederland (2016): 1.230 – 2.295. Daadwerkelijke volume is moeilijk te voorspellen. Correctie voor performance status en positieve PD-L1 zal potentiële patiëntenpopulatie drastisch reduceren. Daarnaast is er nog geen eenduidige afkapwaarde voor positieve PD-L1 expressie gedefinieerd en is er sprake van hoge interobserver variabiliteit. Volume wordt ook beïnvloed door de overleving in de 1e lijn, de performance status, leeftijd als proxy, gebruik van combinatietherapieën. 780 patiënten die in aanmerking komen, 156 patiënten die in het eerste jaar behandeld zullen worden. NKR 2016: 7728 diagnoses NSCLC, stadium 3 en 4. 35% niet-plaveiselcel; 60% ontvangt eerstelijnsbehandeling, waarvan 50% chemotherapie, waarvan 50% tweedelijns immuuntherapie. 23% plaveiselcel; 60% ontvangt eerstelijnsbehandeling, waarvan 70% chemotherapie, waarvan 50% tweedelijns immuuntherapie. In totaal zijn dit samen 406+373=779 patiënten die in aanmerking komen voor atezolizumab. Realistisch volume: 156 patiënten die in het eerste jaar behandeld zullen worden op basis van 20% marktaandeel.
