Additional remarks Positieve CHMP opinie ontvangen in januari 2022. Avapritinib is een nieuw intramuraal geneesmiddel. Op 24 maart 2022 heeft de Europese Commissie een handelsvergunning afgegeven voor een nieuwe indicatie van avapritinib. Het betreft de behandeling als monotherapie van volwassen patiënten met agressieve systemische mastocytose (ASM), systemische mastocytose met een geassocieerd hematologisch neoplasma (SM-AHN) of mestcelleukemie (MCL), na tenminste één systemische therapie. Deze aandoeningen vallen onder de overkoepelende indicatie: geavanceerd systemische mastocystose (AdvSM). Dit middel is voor deze indicatie in de sluis geplaatst.

Therapeutic value

Current treatment options Midostaurine, Mestcel-eradicatietherapie, Interferon-alfa in combinatie met prednison, Imatinib, Cladribine. Voor agressieve varianten van mastocytose kan mestcel-eradicatietherapie geïndiceerd zijn. In uitzonderlijke gevallen is allogene stamceltransplantatie een optie, meestal als er een bijkomende hematologische maligniteit is.

Additional remarks In PATHFINDER (NCT03580655) werd de werkzaamheid van AYVAKIT bij de behandeling van AdvSM gebaseerd op het totale responspercentage (ORR) bij 53 patiënten met AdvSM. De patiënten zijn gedoseerd met maximaal 200mg per dag. Avapritinib werd continu toegediend in cycli van 28 dagen tot progressie, intolerantie of ontwenning. Voor alle evalueerbare patiënten was de mediane duur van de respons 38,3 maanden (95% betrouwbaarheidsinterval).

Additional remarks Systemische mastocytose is een zeldzame ziekte die wordt veroorzaakt door een overmatige deling van afwijkende mestcellen. Door de grote variatie aan symptomen en de relatieve onbekendheid met de ziekte wordt de diagnose vaak laat gesteld. De incidentie in Denemarken wordt geschat op 0,06 per 100.000 inwoners. Volgens het Nederlandse expertisecentrum voor Mastocytose is de incidentie ongeveer 15 patiënten per jaar, plus een prevalent cohort van 20 patiënten (3).
