Substantiation Op basis van de beschikbare gegevens is nog geen goede getalsmatige onderbouwing te geven. Er zijn geen studies met Maapliv, wel zijn er studies met parenteraal BCAA-vrije aminozuren bij MSUD patiënten. De risico's van een metabole ontsporing en irreversibele hersenschade bij MSUD zijn zeer groot. Daarom komen jonge patiënten in toenemende mate in aanmerking voor levertransplantatie. Theoretisch zou er therapeutische waarde van Maapliv kunnen bestaan onder specifieke, acute omstandigheden bij niet-getransplanteerde MSUD patiënten. Maximum patient volume for sluice References 1: Expertopinie; Additional remarks Intraveneuze toediening van BCAA-vrije aminozuren is alleen nodig bij ernstige ontregeling waarvoor opname nodig is en orale behandeling niet lukt (33%), weigering van een neussonde (31%), spoedgevallen (14%) of comapatiënten (8%) en als profylaxe vóór een operatie (6%). Aantal decompensaties/operaties 0,25 per jaar. Op basis van de prevalentie (± 30) waarschijnlijk maar de helft die ontregelt, waarvan dan een fractie (5 tot 8) opnames per jaar waarvoor intraveneuze behandeling nodig is. Met de hielprik in Nederland waren er drie patiënten in de afgelopen zeven jaar, waarvan er twee een levertransplantatie hebben ondergaan en de derde gescreend wordt. Omdat na diagnose door middel van de hielprik levertransplantatie bij jonge kinderen een reële behandeling is cq. behandeling van voorkeur, zal het patiënten volume op langere termijn waarschijnlijk eerder kleiner worden (1).
