Current treatment options BEACOPPesc en ABVD zijn beschikbaar voor stadium IV HL.

Substantiation De 6-jaar overall survival uitkomsten waren 93,9% (95% CI, tegenover 91,6 tot 95,5) in de A+AVD groep en 89,4% (95% CI, 86,6 tot 91,7) in de ABVD groep. Progressie-vrije survival was langer met A+AVD dan met ABVD (HZ, 0,68; 95% CI, 0.53 to 0.86). De resultaten zijn vooral behaalt bij de stadium 4 patiënten in de studie, hier is echter al een indicatie voor. Bij de stadium 3 patiënten zijn de resultaten minder. HR stadium IV is 0,48 vs 0,86 in stadium III. Mogelijk is er een plaats bij een kleine groep stadium III patiënten waar ABVD niet ingezet kan worden vanwege de longtoxiciteit.
Dosage per administration 1,2 mg/kg V infusion on Days 1 and 15 of each 28-day cycle.

Additional remarks Incidentie van Hodgkin lymfoom in Nederland was in 2021: 498 patiënten (1). Tot 30% van de patiënten met stadium III of IV Hodgkin’s lymphoma hebben refractaire ziekte of relapse na frontliniebehandeling met ABVD (2). Van de patiënten heeft 36% stadium III en 64% stadium IV. Er wordt verwacht dat een kleine groep van ongeveer 20 stadium III patiënten waar ABVD niet ingezet kan worden vanwege de longtoxiciteit in aanmerking komen (3).
