Stamceltransplantatie is voorlopig namelijk de enige curatieve optie. Over het algemeen wordt door de kinderarts-hematoloog gestart met ijzerchelatietherapie bij een ferritine gehalte vanaf 1.000 microgram/l. Afhankelijk van het type thalassemie kan overwogen worden om hydroxycarbamide therapie te starten om de transfusiebehoefte te verminderen of te voorkomen. Bij toenemende transfusiebehoefte ten gevolge van hypersplenisme kan een splenectomie de transfusiebehoefte verminderen. Hierbij moet echter het risico op trombose meegewogen worden. Daarnaast zijn therapeutische interventies noodzakelijk om ijzerstapeling zoveel mogelijk te voorkomen (ijzerchelatietherapie) als ook medicamenteuze therapie om de gevolgen van ijzerstapeling te verminderen, zoals calciumcarbonaat ter bestrijding van osteopenie.
